New Patient Information

Watada Wellness is a member of the Integrated Wellness Modalities Association. This association was formed to provide alternative wellness services to clients without the normal overhead of other similar providers.

In order to receive services from Watada Wellness, you need to become a member of the Integrated Wellness Modalities Association which will allow you to expand your choices and give yourself the opportunity to choose from a wide variety of techniques, tools, and products that are not available otherwise.

Please print out and bring the following two forms to your first appointment:

What to expect during your first visit:
When you arrive for your first appointment you will be asked to complete paperwork (if not already finished) and we will have a short interview about your reason for visiting. You will then be assessed by the practitioner who will then perform gentle movements on your body. You may feel very relaxed and fall asleep during the session. 

Allow up to 75 minutes for your first session and 30-60 minutes for each following appointment. Please arrive for your appointment wearing loose fitting, comfortable clothes (avoid denim) as all the work will be performed over your clothing. Be sure to drink ample water prior to your appointment so you are well hydrated.

Cancellation and No Show Policy
Our goal is to provide quality care. We understand that sometimes unexpected delays and events can occur. No-shows, late arrivals and last minute cancellations inconvenience us and take away from others that could be seen instead. Please call as soon as possible if you know you need to change or cancel your appoinment. A no-show is when you miss a scheduled appoinment without cancelling and a missed appointment fee will be charged.
